My summarised electronic health record
What is it ? What’s in it for me?
It is your summarised health record which contains essential information on your health status. Healthcare providers also refer to it as a “SumEHR” which is the English acronym for “Summarised Electronic Health Record”.
It is created by your doctor and allows other healthcare providers to see, at a glance, all the most important information on your health It is important if you find yourself in the emergency department, but also to ensure the quality and proper implementation of all care provided to you.
Your summarised electronic health record is an evolving document, ideally it should be updated and supplemented according to your health status at each visit to your general practitioner.
The benefits of the summarized electronic health record :
- Enables the best possible treatment to be given to patients
- Makes the patient the actor of his health
- Facilitates the sharing of information between health professionals
- Helps in the care of the patient
- Useful in case of emergency
- Is a medical history of the patient
What are the different sections of the SumEHR ?
Your general practitioner does not share your entire patient record. The doctor only publishes the information that is relevant to your continuity of care.
The summarised electronic health record has 11 tabs. The extent to which these tabs are completed will depend on your health status.
The tabs
- General overview: contains the administrative data relating to the patient
- Patient wishes: this details the end-of-life wishes of the patient in the event that you are no longer able to express your wishes clearly. These wishes differ from person to person. Specifically for this reason, it is important to indicate what you want and do not want clearly (for example: resuscitation, organ donation, intubation, futile medical care, euthanasia request, clinical trials, etc.). These choices are not definitive, you are able to modify them as you wish.
- Allergies
- Drug allergies: reactions to medicines
- Social risks: your current life situation that may have an impact on your health
- Risk factors: things that might complicate the management of your health
- Conditions (sometimes also known as diagnoses): this relates to current conditions that are important to detail
- Treatments
- Current medications: medications that you are currently taking
- Previous medications
- Vaccinations: your doctor can record the dates of vaccinations and doses received
Who can access my SumEHR ?
- Your general practitioner can create your summarised electronic health record and put it online. This is done with your agreement (if you have consented to the sharing of health data).
- Other healthcare providers treating you and with whom you have a “therapeutic relationship” can then consult it in full or in part depending on their profession.

If you have consented to the sharing of your health data, log in to the portal by following the steps below :

- Log in to the Brussels Health Network and click on the button “patient connection”
- Authenticate using itsme or your eID card
- Click the tab “My Data”
- Then on the tab “SUMEHR” and open your summarised health record
I have a SumEHR but it is not up to date
Only the author of the document (therefore your general practitioner) can make changes to your record.
- During your next consultation, ask the doctor to make any required updates
- You can also use the online brochure for any support you may need
I don't have a SumEHR
- Speak to your doctor and ask them to create one for you

- It may be that your doctor has not published the record on the network. The doctor can contact the Brussels Health Network to find out how to do it: we offer free training and tools to assist in the process
